Pregnancy is a special case: hormonal fluid shifts crowd the tunnel, and symptoms can appear in the later months. These are suggestive but not definitive on their own.
The most reliable test is a nerve conduction study, sometimes combined with electromyography. It measures how well and how fast the median nerve is transmitting signals across the wrist, confirms the diagnosis, grades the severity, and rules out other nerve problems that can mimic it. When surgery is the answer
Surgery becomes the sensible choice when numbness is constant, when the thumb muscle is weakening, when nerve studies show significant compression, or when good conservative treatment has failed to control symptoms.
